As a Manager of Engineering, Maintenance and Safety I was always challenged to create real awareness of the hazards encountered during routine safety meetings. This article is intended both for safety professionals and also for use by supervisors as a start to discussion for individuals in these routine safety meetings. For each class of underlying hazard we briefly describe a few recent cases as illustrations and then discusses the common patterns of behavior (latent, systemic deficiencies) as well as the specific causes of these predominantly fatal accidents.
